Gillette Children’s Health Information Management (HIM )Department is committed to respecting and protecting the rights of our patients and families. The HIM staff is responsible for maintaining the confidentiality of all patient records and can help you with your medical records requests.  

Parents, guardians or patients have the right to review and receive a copy of their child's medical records when authorized. Please note that the HIM team cannot interpret what the information means or discuss it with you. 

If you have additional questions after reading the information below, you may contact the HIM Department at 651-229-3886 (option 3) from 8:00 a.m. to 4:30 p.m. Monday through Friday, excluding holidays. 

 

Your Medical Records

Access portions of your child's medical record by accepting an email invitation to enroll in the  MyGillette patient portal. If you do not have access to the MyGillette portal, you can request access here

For medical records not available on the portal, refer to the next section. 

Request medical records via secure website hosted by our release of information partner, Ciox by completing the form here. If you are unable to complete your request online, refer to additional options to submit your medical records request, or contact the HIM team.

Once the HIM team receives your request, it will take approximately 5 to 7 business days for the records to be mailed to the address you provided.  

Gillette Children’s will grant the patient the right to amend their Protected Health Information (PHI) according to federal and state regulations. 

If you are requesting an amendment or correction to your or your child's PHI, complete the PHI Amendment/Correction Request form and send via email to  or mail it to the address listed above.

Once you have completed and submitted the request for an amendment or correction the HIM team will respond to your request within 60 days by: 

  • Processing the requested amendment 
  • Providing the patient with a written denial/approval 
  • Providing the patient with a written statement of the reasons for the delay and the date by which Gillette Children’s will respond to the request (which date will be within 90 days after receiving the request).
