The 2026 Gillette Children’s Pediatric to Adult Health Care Transition Summit will take place Monday, May 18, 2026, from 9:00 a.m. - 4:00 p.m. at the Wilder Foundation. The overall theme of the 4th Annual Summit is Connection: Humanizing the Health Care Transition (HCT) from Pediatric to Adult Serving Systems.

After attending, participants will be able to:

  • Define resilience in the context of life changes during the health care transition period, from youth, family, and clinician perspectives.
  • Apply narrative medicine to humanize the healthcare transition experience.
  • Describe how local Minnesota organizations implemented evidence informed health care transition programs.

Transform Your Practice to Support Pediatric to Adult Transition

Registration is now open for the 2025 Gillette Children's Health Care Transition Learning Collaborative ECHO Series!

The Minnesota Health Care Transition Learning Collaborative (HCT LC) is a groundbreaking project aimed at advancing pediatric to adult health care transition for children and youth with special health needs. By attending the ECHO sessions, participants will examine evidence-informed pediatric to adult health care transition practices and case presentations and translate pediatric to adult health transition learning and peer-to-peer support to clinical practice.

The series is open to anyone interested in transforming pediatric to adult health care transition.

Topics for the ECHO series include:

  1. Promoting self-advocacy and independence during Health Care Transition (HCT) on October 6th, 2025, from 12:15 – 1:15 PM CST
  2. Putting the Good in Goodbye during Health Care Transition (HCT) on December 1st, 2025, from 12:15 – 1:15 PM CST
  3. Addressing Ableism and Health Equity during Health Care Transition (HCT) on January 26th, 2026, from 12:15 – 1:15 PM CST
  4. Preventative Care for Adolescents/Young Adults (AYAs) during Health Care Transition (HCT) on March 23rd, 2026, from 12:15 – 1:15 PM CST 

We are delighted to welcome Dr. Patience White and Peggy McManus, from Got Transition, a program of the National Alliance to Advance Adolescent Health. You will learn from mental health, education, healthcare, school nursing, family advocacy and support, vocational rehabilitation, and employment experts that successfully support youth as they transition to adult services.

Dr. White and Ms. McManus, along with clinical experts from Gillette Children’s and other esteemed guests, will provide expertise and guidance to foster engaging dialogue centered on current conversations in pediatric to adult HCT.

CME Credit Designation (for each session): 1.0 AMA PRA Category 1 Credits™ | 1.0 Contact/Clock Hours

Physicians: Gillette Children’s is accredited by the Minnesota Medical Association (MMA) to provide continuing medical education (CME) for physicians and takes responsibility for the content, quality, and integrity of this educational activity.

Gillette Children’s designates each session of this live activity for a maximum of 1.0 AMA PRA Category 1 Credits™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity. 

Nurses: This activity is designed to meet the Minnesota Board of Nursing (MNBON) continuing education requirements for 1.0 contact hours. It is the participant’s responsibility to ascertain if this activity meets the mandatory continuing education requirements of the licensing board under which they practice.
