Understand Your Rights and Responsibilities

When you come to Gillette, know that we comply with applicable federal civil rights laws and don’t discriminate based on race, color, national origin, age, disability, or sex. Download our Nondiscrimination Standards (PDF).


Request Your Medical Records

Copies of your medical records are available from Gillette Children’s Health Information Services department.  You can also view health record in our online patient portal, MyGillette.

To request medical records, you as a patient, or as a parent or guardian of a patient, must complete and sign an Authorization to Release form.

If you are unable to print the form, or have questions about requesting medical records, please call 651-312-3122.

Acting as a written request, your completed Authorization to Release form:

  • Helps Gillette correctly identify the medical information you want to receive.
  • Documents your authority to receive information you are requesting on a patient’s behalf. Proof of guardianship is required to be on file in the patient's medical record to obtain medical records.
  • Provides Gillette with a record of written requests and the medical information we’ve released.
  • Is necessary to comply with federal and state regulations and laws.

Notice of Privacy Practices

Your health information—about you or your child—is personal. Protecting the privacy of your health information is critical. Please know that Gillette complies with all applicable federal and state privacy and confidentiality laws.

The Notice of Privacy Practices describes how we maintain your privacy. It describes personal information about patients, how we use it and with whom we share it. This notice also explains your right to not have your information shared with parties outside Gillette.

If you have a concern regarding your privacy or questions regarding this notice, please call 651-578-5632.

We help ensure your privacy by releasing your personal information only to people you've agreed should receive it. If you would like to authorize disclosure of your personal information to someone, such as a family member or attorney, you may contact our Health Information Services department at 651-229-3886.
