When you come to Gillette, know that we comply with applicable federal civil rights laws and don’t discriminate based on race, color, national origin, age, disability, or sex. Download our Nondiscrimination Standards (PDF).
Request Your Medical Records
Copies of your medical records are available from Gillette Children’s Health Information Services department. You can also view health record in our online patient portal, MyGillette.
To request medical records, you as a patient, or as a parent or guardian of a patient, must complete and sign an Authorization to Release form.
- Authorization to Release (PDF).
- Authorization to Release (PDF) Spanish.
- Authorization to Release (PDF) Hmong.
- Authorization to Release (PDF) Somali.
If you are unable to print the form, or have questions about requesting medical records, please call 651-312-3122.
Acting as a written request, your completed Authorization to Release form:
- Helps Gillette correctly identify the medical information you want to receive.

- Provides Gillette with a record of written requests and the medical information we’ve released.
- Is necessary to comply with federal and state regulations and laws.
