Transforming Care and Experience for Children with Health Complexity

A conference for care coordinators, specialists, parents, pediatricians, primary care physicians, advanced practice providers, nurses, as well as other interested health care professionals. Our overall conference goal is to advance the system of comprehensive, family-centered care coordination that supports families of children who have complex conditions.

Course Objectives

At the completion of the course, participants should be able to:

  • Promote shared plans of care as a tool to increase coordination of care across the child’s providers and family
  • Identify how to support families and providers to work as colleagues in the child’s care
  • Recognize services and support that families need to successfully care for child in the home

Accreditation

Gillette Children’s is accredited by the Minnesota Medical Association to provide continuing medical education (CME) for physicians. Gillette takes responsibility for the content, quality and scientific integrity of this activity.

Gillette designates this live educational activity for a maximum of 7.0 AMA PRA Category 1 Credits . Physicians should only claim credit commensurate with the extent of their participation in the activity.

This activity is designed to meet the Minnesota Board of Nursing continuing education requirements for 7 contact hours. It is the responsibility of the participant to determine if this program meets Minnesota Board of Nursing rules relating to mandatory continuing education provisions of the Minnesota Nurse Practice Act.

This activity is worth a maximum of 7.0 clock hours. It is the responsibility of each individual to determine if this activity meets their professional requirements for continuing education credit.

Registration Information

The conference registration fee is $50 for healthcare professionals and $25 for parents/family members. Parent scholarships are available upon request. The fee includes parking voucher, continental breakfast, lunch, afternoon refreshments, online conference materials and continuing medical education credit.

Featured Speakers

David Bergman, MD, is an associate professor at Stanford University School of Medicine. His research has been focused in the areas of practice guidelines and quality improvement, the redesign of clinical services, and the use of Web-based patient portals to treat chronic illnesses and the resulting impact on doctor–patient communication. Dr. Bergman serves as chairman of the board of the National Initiative for Children's Healthcare Quality and is a former chairman of the American Academy of Pediatrics Committee on Quality Improvement. He is also a member of the executive committee for Child and Adolescent Health Measurement. Dr. Bergman remains clinically active and is actively involved in providing well-child care to children. He received an MD from the University of Illinois School of Medicine.

Jeanne W. McAllister, BSN, MS, MHA, is an Adjunct Research Associate Professor of Pediatrics at the Indiana University School of Medicine, division of Children’s Health Services Research. She is the Co-Founder and former Director of the Center for Medical Home Improvement (1997-2013), a health care consultant and frequently sought-after speaker, a CMMI Innovation Fellow/Advisor (2013).  Ms. McAllister is lead author or co-author of numerous articles focused upon medical home improvement and measurement, care coordination, team-based care, shared plans of care, and health care transition.  Ms. McAllister’s background is in clinical health care, health care administration, health professions education, quality improvement and health services innovation. She blends these distinct areas to improve systems of care with/for children with special health care needs and their families. Care coordination has the potential to bring cohesion to fragmented care systems.  Ms. McAllister’s most recent work has been the development and evaluation of an evidence-based care coordination program using a shared plan of care as a driver of innovative care integration for children with special health care needs.

Bill Schwab, MD is Professor and Vice Chair of the Department of Family Medicine and Community Health at the University of Wisconsin School of Medicine and Public Health where he is an active clinician and teacher.  He is a nationally recognized leader in patient- and family-centered care as well as in medical education about the care of children and adults with chronic illness and disabilities and has led research projects on developmental screening in primary care and on the care of individuals with autism.   He has been the recipient of the Humanism in Medicine Award from the Society of Teachers of Family Medicine jointly with the Arnold P. Gold Foundation, and the Educator of the Year Award from the Wisconsin Academy of Family Physicians. Bill serves on the board of directors of the Institute for Patient- and Family-Centered Care and of Family Voices of Wisconsin. 

Jennifer Sommerness, EdS brings her belief and passion in the power of inclusive education to the Kids Included Together leadership team as the Executive Director, Inclusive Schools. Prior to joining KIT, she was a nationally-known disability inclusion independent consultant working with teachers, families and administrators to collaboratively find engaging and respectful ways for all learners to access general education environments, regardless of ability. As an adjunct professor at the University of St. Thomas in Minnesota, she taught aspiring teachers the foundational skills required to instill inclusive practices into their profession. As an independent consultant, she worked at the practice level to help create and strengthen school-based teams to continuously improve their practice through meaningful professional development, attention to instructional leadership, and a tacit understanding of systems change. In addition, she has collaborated and co-authored several peer-reviewed journal articles, a book chapter, curricula and online materials about collaboration, co-teaching, teacher leadership and inclusive education. Jennifer joined KIT because of their progressive approach to teaching disability inclusion and positive behavior support for over 20 years. She brings her passion for inclusive education and over 25 years of experience to help KIT achieve their vision of all children realizing their full potential in a welcoming world.
